Commendation chests: Its rare but possible for commendation chests to contain red items, so even a simple level up can help you get a veteran item.Legend difficulty vaults: I find it much easier to get veteran tier items on legend since you don’t even need tomes and grims, the lowest level vaults still have a chance to give out red items.Top tier champion chests: As far as I'm aware, you can get red items on champion difficulty, but you'll need to gather tomes and grimoires to get the better chests.

These cherished red items are difficult to get because chance is a primary factor, there is no way to guarantee you’ll get one since you cannot even upgrade an exotic(orange) item to get to the red tier, chests are the only way to get them.
